The Dog Lady L.L.C. — Client Training Agreement & Liability Waiver
CLIENT RESPONSIBILITIES
The Client agrees to the following conditions:
• To follow the Trainer’s instructions without modification regarding all handling, training techniques, and behavior expectations.
• To pay the total amount quoted for agreed-upon training services. Payment terms must be confirmed at the time of booking.
• To provide a minimum of 24 hours’ notice in case of cancellation. Failure to do so may result in full payment for the missed session.
• To provide training treats and have their dog arrive in approved training equipment, such as a flat collar, prong collar, harness, or slip lead, based on trainer recommendation.
• To provide proof of required vaccinations, including Rabies, DAPP, and Bordetella, before any training begins.
TRAINER RESPONSIBILITIES
The Dog Lady L.L.C. agrees to provide:
• Professional training services tailored to the dog’s needs and learning pace.
• Group sessions, private lessons, day training, or equipment-based enrichment, depending on the agreed-upon program.
• Take-home worksheets and structured follow-up to support continued learning.
• Guidance on rules, expectations, and relationship-building between dog and owner.
• Safe and humane handling using industry-standard tools and equipment.
DAY TRAINING PROGRAM
The Client understands and agrees that Day Training consists of 2–3 hour drop-off sessions in which the Client brings the dog to the Trainer’s facility. The Trainer does not provide transportation. Day Training may include:
• Socialization and leash work in public areas such as Downtown Farmington, T.J. Maxx, Ace Hardware, and other pet-friendly businesses.
• Structured exposure to controlled distractions and environments.
• Desensitization in the grooming room to tools and handling (see grooming tools section below).
• Rest breaks in a secure crate or kennel between activities.
Dogs must be people- and dog-safe for public sessions. The Trainer reserves the right to modify or limit activities based on behavior, weather, or other safety factors.
MOTION LAB: SLATMILL EXERCISE
The Client understands that participation in Motion Lab sessions may include use of a slatmill (a non-motorized, dog-powered treadmill) under the direct supervision of a trained professional.
Dogs are never forced onto the slatmill. Introductions are gradual, and each session is customized to the dog’s age, temperament, physical health, and experience. Dogs may be guided using leash cues, voice, or food motivation as appropriate. Breaks are provided as needed, and dogs are carefully monitored for signs of fatigue or stress.
Client Acknowledgement:
I understand that Motion Lab sessions are physically engaging and may involve cardiovascular exertion. I confirm that my dog is in good physical health and cleared for activity. I acknowledge and accept the risks involved in slatmill use and agree not to hold The Dog Lady L.L.C. liable for any injury or discomfort my dog may experience as a result of Motion Lab participation. I have been given the opportunity to ask questions and decline participation if I choose.
EQUIPMENT & TOOL USE DISCLOSURE
I understand that The Dog Lady L.L.C. may use a variety of training and grooming tools, including but not limited to: crates, slip leads, prong collars, e-collars, grooming tools (such as nail clippers, dremels, and brushes), and slatmills. These tools are used professionally, humanely, and in alignment with my dog’s individual behavior plan. I acknowledge I have been given the opportunity to ask questions about any tools used and agree to their responsible use in my dog’s training and grooming experience.
EMERGENCY MEDICAL CARE
In the event of illness or injury during a session, I authorize The Dog Lady L.L.C. to transport my dog to a veterinarian or emergency care provider as deemed necessary. I understand that reasonable efforts will be made to contact me prior to treatment. I accept full responsibility for all veterinary costs incurred and agree that the safety and welfare of my dog will be prioritized in such circumstances.
PUBLIC OUTINGS LIABILITY
I understand that my dog may visit public spaces and businesses during Day Training. I assume all risks associated with these outings and agree to release and hold harmless The Dog Lady L.L.C., as well as any third-party establishments visited, from liability related to my dog’s behavior, injury, or damage during public sessions.
ENVIRONMENTAL EXPOSURE WAIVER
I understand that my dog may be exposed to a variety of environmental conditions during training, including but not limited to: heat, cold, snow, rain, uneven surfaces, traffic, and distractions such as people, dogs, or noise. The Trainer will take all reasonable precautions to ensure safety, but I accept these risks and do not hold The Dog Lady L.L.C. liable for any injury or stress caused by environmental exposure.
PHOTOGRAPHY & MEDIA RELEASE
I authorize The Dog Lady L.L.C. to take and use photos or videos of my dog or myself for training documentation, educational purposes, or marketing content, including but not limited to social media platforms.
LIABILITY WAIVER
I understand that participating in training services with The Dog Lady L.L.C. involves inherent risks, including potential injury to myself, my dog, or others, and damage to property. I voluntarily assume all risks and agree that The Dog Lady L.L.C., its trainers, employees, and affiliates shall not be held liable for any injury, loss, or damage arising from participation in any training activity, including but not limited to: private lessons, group classes, day training, Motion Lab sessions, grooming tool use, or public outings.
I release, discharge, indemnify, and hold harmless The Dog Lady L.L.C. from all claims, demands, or liabilities arising out of or in connection with any training or handling of my dog, including injuries to third parties or damage caused by my dog while under the Trainer’s supervision.
ADDITIONAL TERMS
• The Trainer provides professional services but does not guarantee specific behavioral outcomes.
• The Client is financially and legally responsible for any injury or property damage caused by their dog.
• The Trainer reserves the right to refuse or discontinue service if the dog poses a risk to staff, the public, or other animals.
• Dogs showing signs of illness, injury, or contagious conditions may be turned away at the Trainer’s discretion.
